The specimen consists of approximately 9 inches of terminal ileum; two inches at either end appear normal, but centrally fibrous adhesions have caused narrowing and kinking of the bowel. The disease appears to affect serosa primarily, since the mucosa appears normal except at areas where the constrictions have produced some obstruction. Sectioning shows that there are four main points of constriction, in each case associated with a central area, reddish-brown or stained by haemosiderin, measuring a few mm in diameter, and surrounded by dense fibrous tissue.
Microscopic examination reveals endometriosis at each of the four constrictions. Stroma as well as epithelium can be identified, and some of the acini contain bipod. The endometriosis affects mainly serosa and muscularis but it extends as far as muscularis mucosae in one of the sections. The mucosa shows no primary abnormality but in affected segments, lymphoid hyperplasia is well marked. Bowel from either end of the specimen shows only serosal reaction.
