From a male aged 40 years who gave a very vague history of abdominal pains and complained of weakness. At operation no evidence of inflammation was found. The vermiform process was distended and translucent and so completely affected that the contiguous portion of the wall of the caecum had to be resected in order to effect removal. The vermiform process is uniformly distended to some 15 mm. in diameter. It is slightly curved towards its mesenteriole, of a yellowish-grey colour and its serous surface is smooth, pale and appears devascularized. The wall forms a thin membrane and the mucosa is flattened or has desquamated.
