DescriptionHalf of a vertical section of a cystadenoma of the Pancreas.
From a female aged 37 years who first noticed a small mass in the upper abdomen three month previously end for one week had epigastric pain after eating. There was no sickness and in other respects she felt perfectly well, ho had two children aged twelve and four years. There was no history of any illness of note. On examination she was a spare, healthy subject. The tumour felt by the patient lay under a thin linea alba in the mid-epigastrium, This was part of an elongated. lobulated mass which could be felt on deep palpation. It extended from below and. to the left of the umbilicus to the ensiform cartilage. It was slightly mobile from side to side and was not tender. At laparotomy the lobulated cystic tumour displaced the stomach and transverse colon forwards and could be seen extending upwards to the diaphragm behind the gastro-hepatic omentum. On raising the omentum and transverse colon, the lower pole of the tumour reached downwards behind the transverse mesocolon. The lesser sac was opened below the greater curvature of the stomach and both the upper end. lower poles of the tumour were easily freed, It was attached by a narrow pedicle about the size of a thumb nail to the neck of the pancreas There was no line of cleavage and pancreatic tissue was divided piecemeal after applying mosquito forceps close to the tumour. When the tumour was freed and removed the neck of the pancreas and the main duct were found to be severed; apparently the narrow neck was angled forwards into a sulcus on the posterior aspect of the tumour. The ends of the divided duct were closed. by linen stitches and. a few minute bleeding points ligated. Convalescence was uneventful and there was subsequent Improvement of health.
