An ampulla is a cavity, or the dilated end of a vessel, shaped like a Roman ampulla; this was a spherical jug with two handles.
The specimen is from a female aged 38 who was healthy until four months previously, when she began to lose weight. Six weeks later jaundice appeared and gradually deepened, but she experienced no pain. On examination she was slender and the skin was yellow with a slight shade of green. The liver extended to near the umbilicus and the gallbladder was enlarged and tense.
At operation a tumour in the region of the ampulla could be felt through the anterior wall of the duodenum. The contents of the tensely distended gallbladder were aspirated and no stones were present. The grossly enlarged liver showed no evidence of metastases. No enlarged glands could be felt. The duodenum and the head of the pancreas were mobilised and resected with the lower portion of the common bile-duct. The common bile-duct was connected to the highest loop of jejunum; 3 cm lower the stump of the pancreas was implanted; and 5 cm still lower the free end of the stomach was joined to the jejunum. Recovery was smooth and six weeks later her condition was considered to be very satisfactory.
The preparation consists of about 15 cm of duodenum extending from the pylorus, the head of the pancreas and the terminal portion of the common bile duct. The duodenum has been laid open long its convex border. The duodenal papilla is enlarged to form a cylindrical mass about 2 cm in diameter. The surface is covered with normal mucosa except in the region of the orifice where there is an excavated ulcer. A segment has been removed and reveals the ampulla distended by tumour tissue. A white rod indicates the common bile-duct which is much enlarged. The pancreatic duct into which a coloured rod has been inserted is only slightly dilated.
