Adenocarcinoma of the common bile duct from a male aged 54 years.
Patient had a four week history of obstructive jaundice without pain. The specimen is a resected segment of the common bile duct. The tumour encircles the duct and has involved the cystic duct. It is hard and tough to feel and the cut surface whitish fleshy.
The tumour is an adenocarcinoma. There is considerable pleomorphism, and a degree of fibrous reaction.
There is a tumour encircling the common duct and involving the cystic duct. Where the duct wall has been sectioned the tumour presents as a whitish, fleshy mass. A narrow rod has been inserted in the lumen of the tumour. Proximally the common duct appears to be dilated. Microscopic examination shows adenocarcinoma, forming an elongated thickening of the wall, with ulceration. The malignant glands show much irregularity in size, with some dilatation and infolding of their walls.There is some scirrhous reaction - and much chronic inflammatory cell deposit near the outer surface. No evidence, in plane examined, of invasion of the duct wall.
