DescriptionSquamous cell carcinoma of a gall-bladder.
From a woman aged 76 who gave a vague history of dyspepsia and flatulence for many years. She developed a dull ache in the right upper abdomen and had an attack of bilious vomiting. Two months later she herself noticed a swelling below the right rib margin. Under the right subcostal margin a mass could be felt which was movable transversely and vertically. At operation a tumour was found but it was difficult to determine whether the primary site of the tumour was gall-bladder, transverse colon or pyloric region of stomach. As there was no evidence of spread beyond this fused mass a portion of the liver, transverse colon, distal part of stomach and first part of duodenum were resected. The transverse colon and duodenum were later separated and the primary site was found to be the fundus of the gall-bladder which contained numerous calculi. Recovery was uneventful but five months later a recurrent nodule appeared in the abdominal scar.
A V-shaped portion of liver measuring 11.5 cm. by 9 cm. and the gall-bladder are mounted with the under aspect presenting forwards. A portion of the gall-bladder has been removed and this reveals numerous dark, faceted calculi and a tumour filling the fundus. The tumour has permeated the wall of the fundus and extended into the liver to form a mass 6 cm wide which on the under aspect is nodular and its surface rough. The under aspect of the liver and fundus of the gall-bladder were adherent to the transverse colon and first pert of duodenum, both of which were invaded by the tumour.
