From a male aged 33 years, the subject of acquired syphilis and whose Wassermann blood reaction was positive. He had lost weight during a year, had occasional indigestion breathlessness on exertion, swelling of the abdomen during a month and abdominal discomfort and pain for a fortnight There was much ascites, the abdomen was tapped on two occasions and the fluid withdrawn was clear and contained large and small lymphocytes. The liver was not enlarged. Rupture of an oesophageal vein caused profuse, repeated haemorrhage and death ensued within twenty-four hours.
Superiorly the liver is adherent to the diaphragm and the hepatic capsule shows fine velamentous recent fibrinous adhesions over a considerable area. The surface is mottled and coarsely granular. Section shows three large irregular, avascular, homogeneous, gummatous, yellow, somewhat circular areas surrounded by a capsule of fibrous tissue and destroyed hepatic parenchyma. The gummata occupy most of the section and where liver tissue is still recognizable there is venous congestion and a diffuse cirrhosis separates and infiltrates the brown areas. The capsule is not thickened and immediately subjacent to it lie innumerable cirrhotic nodules with atrophied liver cells.
