Clinical history:
This tumour was detected at a mass X-ray, apparently symptomless. Histology confirms the teratomatous nature of the specimen.
Radiology:
A.P. X-ray shows a rounded shadow overlying the 8th, 9th and 10th ribs, projecting from the mediastinum and displacing the left lung.
Lat. This shows the tumour mass in the anterior mediastinum opposite the lower sternum in the characteristic position of a teratomatous tumour.
Description of specimen:
Globular tumour, 9 x 7 x 4 cm. of which a hemisection is mounted. The outer surface is smooth and has shelled cleanly from the surrounding tissue and presents a globular but slightly bossed outline. One or two vessels are coursing over the surface of the tumour between the bosses but vascularity is not a feature. On section the tumour is seen to be cystic in character with solid areas. The cysts are separated by septa of varying degrees of thickness and the cyst walls are thin and translucent. In addition a more solid area is present as one part of the tumour. Within two of the cysts irregular masses are present and have a thick, white, almost chalky-white appearance. In the solid area masses of comparable white material are present. The solid area otherwise is of a fleshy but slightly varigated appearance. The picture is that of a dermoid cyst and the white masses in it would appear to be in the character of a sebaceous material.
Microscopical examination:
The tumour contains a mixture of tissues including spindle cells of leiomyomatous type, indeterminate mesenchymal cells, lymphocytes and lining cystic spaces, intestinal type mucosa. There is a large central necrotic area in the tumour containing indeterminate debris and this is lined by large swollen cells containing fatty globules.
