From a female aged 62 years who was admitted to hospital as she was experiencing jaundice, anorexia and nausea. On clinical examination the liver was noted to be enlarged. At operation distension of the common bile duct and gall bladder and in the head of the pancreas there was a hard mass. A duodeno-pancreatectomy was performed and the patient made a good post-operative recovery.
In the head of the pancreas a small rounded and lobulated tumour, approximately 1 inch in diameter, was found. This was sectioned to exhibit the features of the tumour and consisted of white fleshy material, somewhat translucent in areas, and a number of small cysts. The tumour is well localised and has no definite capsule and appears to show lobular invasion to the surrounding tissue. A probe passed into the ampulla could not be passed into the bile duct.
Microscopic examination shows carcinoma of the pancreas, an adenocarcinoma derived from the duct epithelium. The ducts are of various sizes, some greatly dilated, and lined by proliferating cuboidal and columnar epithelium. In some areas, the malignant lining cells are infolded to form a papillary arrangement. Smaller ducts form more solid strands of undifferentiated cells, with periductal stroma invasion. The lobules with islets are compressed by the tumour but show no evidence of tumour-genesis in the areas sectioned. The stroma in the tumour is abundant and oedematous, with a diffuse scattering of individual malignant cells. The general hyperchromatism obscures mitotic activity.
The X-ray demonstrates a complete block of the common bile duct indicative of an occlusive lesion in the head of the pancreas.
