DescriptionThe specimen consists of the caecum and ascending colon together with the terminal loop of ileum measuring some 10 inches.The pathological changes are present only in the ileal loop which shows gross ulceration with marked thickening of the mucosa and of the muscularis. There has been haemorrhage into the lumen of the gut. The serosa of the terminal ileum shows many fine adhesions and considerable subserous haemorrhage has occurred but these may well have been due to operation manipulation. These changes in the wall of the gut have made the terminal loop a rigid and typically "hose pipe" structure with narrowing of the calibre of the lumen. The mesenteric lymph nodes are enlarged and prominent.
The mucous membrane can be recognised but is ragged and its interstitial tissue is infiltrated with leucocytes of all kinds.The sub-mucous, muscle and sub-peritoneal layers are involved in a chronic fibro-granulomatous type of reaction in which lymphocytes are the commonest cell.The picture is that of a very chronic ileitis.
