From a female aged 52 years who for one year had complained of abdominal pain, mainly after meals, with occasional vomiting of gastric contents Her appetite had been impaired and she had lost much weight. Gastric analysis showed entire absence of free hydrochloric acid. At operation the diverticulum was found at the usual site in the ileum about 1 metre from the ileocolic sphincter. It had become invaginated into the ileum and had initiated a small intussusception. There was no other intra-abdominal lesion.
The diverticulum is seen to be invaginated into the ileum which is dilated and somewhat hypertrophied. The mucous membrane over the intussusception is congested and ulcerated. There is no indication of acute obstructive dilatation of the proximal ileum, but doubtless there has been a chronic or recurring obstruction. The base of the diverticulum on palpation is considerably thickened and indurated, and appeared to be the site of a simple tumour Microscopic examination of the tumour, which is riot visible in the specimen, indicated a leiomyoma, probably more of a malformation or hyperplasia than a true tumour.
