The specimen, as removed at operation consists of the distal two-thirds of the stomach and first part of the duodenum, together with the greater and lesser omentum. Only the posterior wall has been preserved in the preparation. An ulcerating carcinoma has surrounded the pyloric antrum and extends proximally from the pylorus involving the whole distal third of the stomach. The tumour projects into the lumen causing some degree of stenosis while ulceration is clearly seen in the more distal part of the stomach (pyloric antrum) proximally the appearance suggests submucous infiltration and no clear line of demarcation between the tumour and normal gastric mucosa is apparent. This is confirmed by the appearance of the cut edges of the specimen, A few enlarged and involved lymph nodes are present in the greater and lesser omenta. - There was no evidence of the previous perforated ulcer.
An ulcerated carcinoma of the stomach, of glandular structure in the mucosal tissue, but infiltrating the deeper coats to the serosal as an undifferential growth with much mucus. There is great congestion of the submucosa and serosal tissues. One of the lymph nodes examined, from the greater omentum, shows invasion by glandular carcinoma.
