Clinical history:
From a male aged 70 years admitted to hospital because of haematemesis. Gastroscopy revealed an area of irregular mucosa and several polypi on the greater curvature in the lower part of the body and upper part of the antrum. A partial gastrectomy was performed.
Description of specimen:
The specimen shows a relatively large polypoid area in the proximal part of the antrum which is smooth-surfaced and not ulcerated. More distally there are a few more discrete rounded polypi. The cut edge of the viscus shows an apparently invaded muscularis and the appearance is that of a superficial carcinoma.
Microscopical examination:
The apex of the major polyp shows a thin, single-layered, glandular layer overlying a very haemorrhagic, inflamed mucosa, in which are irregularly disposed dilated glands with polypoidal ingrowths. These mucosal structures show an early malignant change, with moderate mitotic activity, in some areas.
The area of lobulated thickening of the mucosa shows a generalised overgrowth, with an admixture of normal and malignant glands. The muscularis mucosae is intact, but shows a few small deposits of tumour cells. There is no deeper infiltration in the area examined.
