From a male aged 40, who for many years had suffered from pain after eating. An acute perforation of the ulcer in his stomach demanded urgent operation which improved his symptoms. Eight months after, the pain recurred and had since persisted, growing worse. Operation revealed a large ulcer on the posterior wall of the stomach invading the pancreas. A partial gastrectomy was performed.
The stomach presents a chronic ulcer situated on the lesser curvature and adjacent to the posterior wall of the stomach approximately 5 cm from the pylorus. The ulcer is some 1.5 cm in its long axis and is somewhat saddle-shaped. It has a deep crater with typical overhanging margins. The stomach wall in the vicinity arid the connective tissues of the lesser omentum adjacent are thickened and fibrous as a result of the chronic infection. The mucous membrane adjacent to the crater is drawn into radiating folds as a result of the fibrosis.
