The GC notes state that the patient, a 40 year old man, had suffered stomach pains for 10 years but latterly had a perforated ulcer causing bleeding necessitating transfusion of 5 pints of blood. The upper 2/3 of the stomach, to include the ulcer, were removed and forms this specimen. The preparation consists of the right two-thirds of the stomach an. a small portion of the first part of the duodenum. It has been laid. open along the greater curvature. High on the anterior wall and extending to the lesser curvature there is a deeply excavated chronic ulcer measuring 2 cm. by 3 cm. The edges are smooth and regular. The ulcer is fully 1 cm. deep and is lined by a flat even layer of granulation tissue. On the right side of the base of the ulcer is an eroded vessel of moderate size which no doubt gave rise to the haemorrhage, but which now appears to be thrombosed. The gastric mucosa is very rugose and somewhat congested and oedematous. On the serous surface there is very evident scarring over and around the ulcer.
The ulcer is very evident as a large oval crater in the centre of the specimen with rugae (stomach folds) radiating out from it. To the left and right is normal looking stomach with overly abundant and swollen rugae but around the ulcer the rugae are a diminished and form an unusual pattern radiating out from the ulcer. The cartoon shows how the peptic ulcer is a shallow crater where stomach acid has eaten through the surface layers (the mucosa and sub-mucosa) causing pain. Stomach ulcers are common, usually caused by an infection with a bacterium called Helicobacter pylori although some medicines can cause ulcers. A 4-8-week course of acid-suppressing medication plus two antibiotics will usually clear the Helicobacter infection and allow the ulcer to heal and prevents the ulcer from recurring again.
