From an alcoholic male aged 55 years who had a left acute otitis media four years previously. Local treatment only was adopted, and discharge from the acoustic meatus had continued uninterruptedly since with occasional earache and headache. Having on that account been off work twelve days, he had a convulsive seizure and the right upper and lower extremities, after a series of clonic spasms, became paralysed. He had loss of speech and later of consciousness. After a delay of two days a left radical mastoid operation was performed and the left temporal lobe explored, but no pus obtained. While removing more bone posteriorly at a higher level than the transverse sinus a large quantity of epidural pus was suddenly evacuated. Considerable improvement followed and speech returned, but he was unable to name objects. He died from meningitis seven days later.
The removal of bone has been fairly extensive, and the large irregular opening exposes the epidural abscess cavity, the dural surface being covered with a flocculent seropurulent deposit. The abscess has been very chronic and inflammation has led to decalcification of the inner table with reformation of new bone, thickening this part of the skull and the adjacent part of the calvaria.
